Please provide one of the following documentation for verification when returning this registration form:

• A signed introduction letter by your Sifu/Coach/Instructor (sample format provided below.)
• Picture of you with your Sifu (if deceased) and/or Schoolmates you attended
• Other documentation that proves you belong to that school or organization (e.g. tuition receipts)
• Photocopy of the original registration/admission form of your school/organization with your signature and date.
• Any letter of approval, qualification or certification by the Sifu, school, or organization.

Please note that a verification process, up to 10 days,
will commence upon the receipt of the returned registration
form (must be completed, signed and dated) and the required
documentation by the Association.  An appointed agent will
act on behalf of the Association to contact the applicant and
references. Once application is approved and membership fee
is paid, applicant will be notified by email confirming their
acceptance and the privilege of accessing the website and
other membership benefits.
